PLS Registration Information and Forms
In addition to providing two forms of address verification
each year, a parent/guardian for each student will be required to sign
an address attest letter that the student(s) lives at the noted address
and that Price Laboratory School will be notified of any address changes
within 30 days of any address change.

You need to turn in a physical form if your child is
entering pre-kindergarten, kindergarten,
grade 7,
participating in
athletics, or is transferring into Price Lab School.
